The information gathered is confidential and will be treated accordingly. It is requested in order to assist facilitator in Risk Management planning. Please fill out details of medication fully. (names of medication, dosage, inhalers, etc.)

Discipline Policy

Purpose:

The purpose of this discipline policy is to establish expectations and procedures for behavior within the Homeschool Gumbo LP co-op group. Our goal is to create a safe and positive environment for learning and growth.

General Expectations:

All members of the co-op group are expected to conduct themselves in a respectful and responsible manner. This includes but is not limited to:

1. Treating others with kindness and respect

2. Avoiding disruptive behavior during class or group activities

3. Being punctual and prepared for all classes and activities

4. Respecting the property of others and the co-op group

5. Following all applicable rules and policies

   
   

Disciplinary Procedures:

1. Verbal Warning: If a member of the co-op group is found to be in violation of the general expectations, they will be given a verbal warning by the group leader or co-op teacher.

2. Written Warning: After a verbal warning has been given, the student will be given a written warning and escorted to the disciplinarian/administrator to contact a parent.

3. Suspension: If a member continues to violate the general expectations after parental notification, they may be suspended from attending co-op classes and activities for a period of time determined by the group leader or co-op teacher. The length of suspension will depend on the severity and frequency of the behavior in question.

4. Expulsion: If a member continues to violate the general expectations after suspension or engages in behavior deemed severe or dangerous by the group leader or co-op teacher, they may be expelled from the co-op group.

   
   

Appeals Process:

If a member or their parents/guardians feel that disciplinary action has been taken unfairly or in error, they may appeal the decision to the administrators. They will review the situation and make a final decision on whether to uphold or reverse the disciplinary action.

Conclusion:

We believe that by establishing clear expectations and procedures for behavior within the Homeschool Gumbo LP co-op group, we can create a positive and productive learning environment for all members. We ask that all members of the co-op group review and abide by this discipline policy.

Parent Participation Contract

We, the undersigned parents, understand that our participation in the homeschool co-op program is crucial to creating a Christ-centered learning environment for our children. As such, we agree to the following terms:

1. Mandatory Participation: We are committed to actively participating in the co-op program, attending meetings and classes as scheduled, and volunteering our time and talents as needed.

2. Christ-Like Behavior: We always commit to modeling Christ-like behavior, treating fellow parents, children, and leaders with respect, kindness, and love.

3. Regular Attendance: We understand that regular attendance is essential to the success of the co-op program and commit to attending all classes and meetings as scheduled unless extenuating circumstances arise.

4. Commitment to Require Children's Participation: We understand that our children's participation in the co-op program is mandatory and commit to ensuring that they attend all classes and complete all assignments with respect for their teachers and fellow classmates.

5. Financial Obligations: We commit to paying all fees associated with the co-op program on time and in full, and to fulfilling any financial obligations to the program.

6. Communication: We agree to communicate openly and honestly with our fellow parents, teachers, and leaders, addressing any concerns or issues in a timely and respectful manner.

Student Contract

As a student of Homeschool Gumbo LP  Co-op, I understand and agree to adhere to the following terms and conditions:

1. Christ-like Behavior: I understand that my behavior should reflect the values and principles of Christianity. I will strive to show kindness, respect, love, and compassion to my fellow students, teachers, and leaders.

2. Following Rules: I understand that rules and guidelines are in place for the safety and well-being of all students. I will follow all rules and guidelines set forth by the homeschool co-op and its leaders.

3. Participation in All Classes: I understand that attending all classes is important for my education and the success of the co-op. I will make every effort to attend all classes and participate fully.

4. Respect for Teachers and Leaders: I understand that teachers and leaders are in positions of authority and deserve my respect. I will treat them with kindness, courtesy, and respect always.

5. Consequences for Misbehavior: I understand that misbehavior will not be tolerated and that consequences will be enforced. Consequences may include suspension from the co-op or removal from specific classes.

Medical Consent

I grant permission, if my child is in need, for the staff or designee, who has a current first aid certificate to provide first aid.

Photo Media Release / Social Media

I grant permission for photographs / video to be taken during school activities to be used for school publicity purposes & the yearbook.

Waiver/Release

I affirm and hold all parties and personnel of Homeschool Gumbo LP, and parties officially appointed or approved by the administration faultless and beyond legalreproach in the in any event where as policies, local, state, and federal laws were determined to be followed by said practitioner.
